Information and/or copies of Birth, Death or Marriage Certificates may be requested online from Service Ontario. Information on the process to change your name in Ontario is also available from Service Ontario.
Paper application forms for Birth, Death and Marriage Certificates are available through Customer Service, 1st Floor, City Hall. The application is mailed, with the required fee, to the Office of the Registrar General in Thunder Bay.
